What are chemotherapy and radiotherapy?

Chemotherapy and radiotherapy are common methods of cancer treatment, which tackle cancer in different ways. Chemotherapy is a form of drug treatment that helps to destroy rapidly dividing cells, including cancer cells; however, it can also affect healthy, rapidly dividing cells. This treatment is known as systemic therapy, meaning it affects the whole body and organism; consequently, its side effects can also affect the whole body.

Radiotherapy, by contrast, is a localised treatment in which ionising radiation is used to destroy the tumour. A specific dose of radiation is delivered to the area to be treated, i.e. the area where the tumour is located. The side effects of radiotherapy, such as hair loss or skin irritation, depend largely on the radiation dose received and on which area is affected by the treatment.

How does chemotherapy affect your hair?

Chemotherapy drugs have been developed to destroy rapidly dividing cells, which is how they combat cancer cells. However, hair follicles also undergo intense cell division, and as chemotherapy cannot distinguish between healthy and cancerous cells, the cells responsible for hair growth are also damaged. Chemotherapy therefore causes hair loss because it damages the hair follicles as well. This process often leads to complete hair loss, but partial hair loss can also occur: around 65% of patients who have undergone chemotherapy experience this phenomenon.

Hair loss usually begins a few weeks after treatment starts, typically within 2–4 weeks of the start of chemotherapy, but symptoms may not appear until months later. The scalp, face, eyelashes and eyebrows may be particularly affected, but hair loss can also occur in other areas. Symptoms of chemotherapy include thinning hair, scalp sensitivity and hair loss in other areas as well.

How does radiotherapy affect the hair?

Radiotherapy acts specifically on the treated areas of the body; consequently, side effects – including hair loss – usually occur only in the area where the radiation directly affects the skin’s surface and where the tumour is located. For example, in the case of brain tumours, localised hair loss may occur due to the ionising radiation affecting the scalp, as the treatment can damage the hair follicles.

The extent of hair loss depends on several factors, such as the size of the area being treated, as well as the type and dose of radiation. Hair usually becomes thinner within 1–3 weeks of starting radiotherapy. Hair loss caused by radiotherapy may be temporary; however, following high-dose or prolonged treatment, the hair may not grow back fully, or may only grow back sparser and weaker.

Following radiotherapy or chemotherapy, your hair and scalp may become particularly sensitive, as the hair follicles are subjected to significant stress during treatment. At this time, it is worth paying extra attention to hair care to promote the regeneration of hair follicles and reduce hair loss. It is important to choose gentle hair care products, preferably containing natural ingredients, which do not irritate the scalp or further aggravate the sensitivity caused by the treatment.

When washing your hair, use lukewarm water and avoid vigorous rubbing so as not to cause further damage to the hair follicles.
